What to Bring To Your Appointment

What to bring to your surgery:

Your health insurance card(s)
Photo ID
Co-Payment or deductible (due at time of procedure)
Pertinent information about your medical and surgical history
A list of all current prescription and over-the-counter medications you are taking. Include dosage and frequency.
Pertinent legal documents, such as Power of Attorney, Advance Directives for Healthcare, if applicable.
A driver is required to accompany you to your surgery and remain in the facility until discharge.
